Surgical Fees, Payment and Insurance Information

Our office maintains that every patient is entitled to the highest quality of Oral and Maxillofacial surgical care that can be provided. Your health and well being are our primary concern. Our office appreciates the consideration you must give to the cost of your care. We recognize that your insurance coverage may help you pay for this care. To accomplish this we work hand in hand with you to maximize your insurance reimbursement for covered procedures. If you have any questions please ask our staff, they are well informed and up-to-date. Please bring your insurance information to your initial visit.
For your convenience we accept Visa, MasterCard, Discover and American Express.
Patients Without Dental Insurance:
Full payment of all charges is expected at the time of surgery, unless other arrangements are made prior to surgery.
Patients With Dental Insurance:
It is important for you to understand that our professional services are rendered and charged to you, not your insurance company. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
If Your Surgical Fee is Not To Exceed $400.00 full payment is expected at the time of surgery. Your insurance forms will then be submitted so you can be directly reimbursed by your insurance company.
If Your Surgical Fee Exceeds $400.00 full payment is preferred. If this is not possible, payment of one half of the fee is expected at the time of surgery and insurance benefits must be assigned to our office. You will be notified when we receive payment from your insurance company. If the payment made is greater than your balance we will send you a reimbursement check. If there is a balance, payment will be expected from you within 10 days.
We allow the insurance company 6 weeks for payment from the time of your surgery. If payment has not been received, payment will be expected from you. Please contact your insurance company if you have not heard from them after 4 weeks to determine the status of your claim.
